 (1)  EAHN member Conference registration includes: participation in all sessions on 31 May and 1 and 2 June / receipt of conference materials (excluding proceedings)/ coffee breaks and lunches on 31 May and 1 and 2 June / reception on 1 June (if indicated when registering).  To be eligible for this reduced registration fee, you must have been registered as a paying EAHN-member before December 2011. Membership status will be verified against the EAHN list of members on the day of registration.
(2)  Non-member Conference registration includes the same as above including one year EAHN-membership.
(3) The Student Conference registration fee includes the same as for members but excluding the lunches. Lunch tickets can be purchased separately at the cost of 15 €.  A signed letter from the Head of Department will be required to confirm the training status. If this letter is not received together with the registration form, the non-member registration rate will be applied.
(4) Conference dinner reservations are limited and will be considered on a first-come, first-served basis. Participants reserving a dinner ticket will receive a voucher in their registration materials.
(3)  Conference Proceedings are not included in any of the conference registration fees and must be ordered upon registering. Extra copies can be purchased at the same rate.
 

OPTIONAL TOUR PROGRAMME
The organizers offer two types of tours: (1) the Lunch Tour Programme on 1 and 2 June and (2) the Post-Conference Day Tour Programme on 3, 4, 5 June. Each tour will be guided in English by an expert guide. The number of participants per tour is limited (indicated below). Registration will be considered on a first-come, first-served basis. On the other hand, the organizers withhold the right to cancel tours which do not have sufficient participants. In this case, participants will be informed and given the option to switch to another tour or receive a refund.

All these tours are walking tours. Tours run simultaneously per day. A packed lunch will be provided upon departure at the conference venue.

PAYMENTS
Payment must accompany the registration. Registration is only final after receipt of payment. Only payments in EUR are accepted. It is possible to pay :
– by credit card (Mastercard or Visa – please note that other credit cards will not be accepted)
– by direct bank transfer into the Momentum / EAHN’12 account with KBC Bank, Heverlee Park, account n° 743-0336632-56, IBAN BE20 7430 3366 3256, BIC/SWIFT code KREDBEBB. For ease of processing, please indicate clearly the name of the participant and the conference EAHN’12 in all your remittances.
 

CONFIRMATION OF REGISTRATION
A confirmation of registration will be sent to each participant, together with an invoice/receipt.
 

INSURANCE 
Please arrange your own travel and personal indemnity insurance.
The organizers will not be liable for accidents, theft and property damage, or for delays and/or modification of the progam due to unforeseen circumstances.
 

CANCELLATIONS AND REPLACEMENTS
Cancellations of confirmed registrations are subject to a charge of 25 € per participant provided the cancellation is received in writing by 31 March 2012.  After this date no refunds will be made. Replacements of confirmed registrations are subject to a charge of 10 EUR per participant.
